The physical includes a review of your athlete’s health history, vital signs and vision, and a hands-on physical examination with a musculoskeletal screening. The FHSAA form is completed and signed at the visit. If anything needs follow-up with another provider, we will tell you before your athlete is cleared.

How much does a sports physical cost?
Sports physicals are offered at a special rate, lower than a standard new patient exam. Call (689) 332-9482 for current pricing.
Can my athlete get a chiropractic adjustment at the same visit?
Yes. If your athlete has back, neck, or joint complaints, a chiropractic visit can be added to the same appointment for an additional fee.
Which form do you complete?
We complete the Florida High School Athletic Association (FHSAA) pre-participation physical evaluation form. Bring the form from your school, with the parent or guardian section filled out and signed.
Does a parent need to come to the appointment?
Yes. A parent or guardian should attend with any athlete under 18.
What payment options do you accept?
We accept major credit and debit cards and CareCredit, a health care credit card that lets you pay over time, subject to credit approval. Apply for CareCredit. Members of the ChiroHealthUSA discount network receive reduced fees.
Do you see patients in Spanish?
